Board of Governors Fee Waiver Form - Application to waive enrollment and health fees. Submit to Financial Aid Office.

Request for Refund Form - Request for refund for dropped or cancelled classes. Refer to schedule of classes for deadline dates

General Petition- R25 - Form to appeal any requests that were originally denied.

R71 - Request for Release of School Info - Authorization from student releasing his or her school information

NOTE: All forms except for BOGs must be submitted to the Admissions and Records office.
