Several of the forms below allow direct data input (i.e. typing) into them.  Some forms are in Microsoft Word format, others are pdf files that work with Adobe Reader.  In order to type directly into the pdf files, you need to use Adobe Acrobat Reader version 5.0 or higher.  When possible, please type directly into the forms, save the file, and then email the file to us.  Thanks!
